Les Cheveux Salon and Spa Facility Agreement

Thank you for your recent interest in our spa. We look forward to extending to you a relaxing, fun filled evening during your time here. Please be assured we will do all we can to make you feel relaxed, refreshed and renewed by the end of your day/evening. Please carefully read the following agreement, sign and return/or fax (765-962-9303) it back to us within 24 hours of contacting us.

1. Deposits:

• There must be one responsible person in your party assigned as "captain" for this party who is willing to be responsible for: collecting the deposits, communicating with Les Cheveux, on scheduling food etc.
• A one time $50 facility charge will be assessed to all spa parties and must be paid along with the following:
  • A deposit of 50% of the total services must be made before the spa can be reserved for your party.
  • A $100 per/person minimum and a $500.00 minimum per party is required to server the exclusive use of our spa for the day/evening. (Example: 8 people =$800.00 minimum in services)
  • Cash may be used as a form of payment, however we must have a valid credit card on file to hold reservation.***

2. Food and Drinks:

• *Les Cheveux is an all-inclusive facility; therefore, bringing your own food or beverage of any kind is prohibited. We offer complimentary wine and and champagne with a 2 glass maximum per person.
• If food services will be desired by your party, you may choose ahead-of-time from our healthy spa menu, prepared by local restaurants and caterers. (This is in addition to your services and does not factor in the maximum dollar amount required in services.)

• *A 15% gratuity will be added to your total bill.

4. Cancellation Policy:

• *Les Cheveux is not responsible for the cancellation of guests in your confirmed party. Scheduled services cannot be cancelled or changed at the last minute. Cancellation must be made at least 48 hours in advance. If you fail to cancel within the appropriate time or fail to show for your reservation, your deposit will not be refunded and your credit card will be charged for the remaining balance.

***I understand and agree to all the above policies regarding the exclusive use of Les Cheveux Salon and Spa. If anyone in my party fails to cancel within 48 hour notice or does not show for their reservation, I will lose my deposit and my credit card will be charged the full amount of the reservation.
